Services commonly rendered are injections for diabetes, wound cleansing and continence maintenance. There were 415 day hospital beds serving 93,400 patients in 1998, with an equivalent of 225 patients for each bed, reflecting a grave shortage of such hospital beds. In 1997, it was estimated that about 20 per cent of Hong Kong’s elderly population had at least one form of physical disability and about 50 per cent had at least one chronic illness (Liu and Wong, 1997). Such people are frequent users of hospitals.

The last British governor, Christopher Patten, set up a Commission for Elderly Services in 1994. A report was produced after six months, in September 1994, which included a number of initiatives, such as revised and improved planning ratios for services the redefinition of old age from 60 to 65, the proposal of an old age pension scheme and the establishment of an Elderly Services Division in the Health and Welfare (H&W) Branch (renamed the H&W Bureau after 1997). However, actual reforms were slow in the few years to 1997.

In the case of LTC, the need for such care tends to hinge on two main dimensions: the degree of the older person’s physical and cognitive impairment, and the availability of informal care for him/her (Deloitte and Touche 1997). However while these may be yardsticks to distinguish between demand and need, the needs of older persons often change during the ageing process. It is hence essential to develop a comprehensive long-term plan to cater for the changing needs of the elderly population. The elderly health centres, the community geriatric and psycho-geriatric assessment schemes, and the gatekeeping (need assessment) mechanism recently developed are geared precisely to this end.
