And this is why I believe that, when we are working with these patients, the daily mental hygiene of cleaning up or clearing the countertransference is a crucial task and that it is mandatory, even for the most experienced analysts, to have a confidant, a peer group, or some other outlet where his feelings and doubts can be fully and frankly discussed. When my patient experienced excruciating psychophysical pain during parts of her analysis as, to a lesser extent, I also did, she was in a primitive bad-mother transference to me that totally altered the meaning of her day-to-day existence.

It seems that meaning and trust originate in the early months as the mother begins to feel what the baby is feeling and the baby begins to feel what the mother is feeling; that is, meaning and trust arise in the reliably consistent interpenetration and mutual regulation of affects and gestural communications in the early dyad. Out of this good-enough mutuality the baby develops the capacity to feel both connected to others and yet separate, which is the dialectical paradox of development. These larger issues of attachment and separation, or relatedness and self-identity are mirrored endopsychically in states of consciousness, so that the state of subjective awareness, or "being totally me and so excited that nothing else exists," proclaims an experience of being a vital and alive separate agent, whereas the state of objective self-awareness, or "being Tony's lover," proclaims an experience of being an object for another human being to whom one is attached.

And I thought that, by not carrying or holding a child's memory or representation in a connected way, a parent can actually murder Time for that child and leave him with all the resulting cognitive and emotional difficulties that I have tried to describe. This particular story has a happy ending, for Jeffrey did eventually learn how to learn, how to connect without too much fear, and how to overcome many of the deficits he had been left with. It was only as I finished writing this chapter that I remembered that, when he was engaged to be married, the first present Jeffrey gave his fiancée was a watch—the significance of which completely escaped me at the time.
